Repeat Prescriptions
Order your repeat medication online, and allow 2 working days for us to process it.
Please make your request well in advance and allow 48 hours for processing. Fill the form in with as much detail as you can: requests that are not clear and accurate will be rejected.

Getting your request right
- Give the exact name of each medicine, and its strength
- Take the details from the right-hand side of your last prescription
- Check the spelling before you send it
- Complete every box on the form
- Name the pharmacy if you would like it posted
Only medications listed on your repeat prescribing can be requested here. If an item was prescribed as an acute prescription previously, it may require a GP consultation and your request may be declined.
How much you will be issued
Usually one month’s medicine will be issued, unless the doctor has previously authorised a three-month prescription and no further checks are necessary for safe on-going prescribing. Three months is the maximum you may request without seeing the doctor.
Doctors are obliged to carry out periodic blood tests and reviews. If your prescription is outside its review date you may be given one month’s supply to tide you over, pending arranging your consultation or review.

This form cannot arrange an urgent or same-day prescription. Phone the surgery and reception will help.
