New Patients
Join the practice as a new patient. Download the form that fits, complete it in full, and send it back to us.
You are welcome to register with us whether or not you hold a health card. Entitlement to a card comes from the Department of Employment, Social Security and Housing, and depends on having lived in the Island for six months or more. Without one, you pay the full cost of your consultation.

Send your form back to us
Post your completed form to Health Plus, Queens Road Health Centre, Queens Road, St Helier, Jersey JE2 4HY. You can also email it to healthplus@healthplus.gpnet.je, or fax it to 01534 762957.
Who you will see
You will be registered as a patient of the surgery. Our registrations team will be able to explain which doctors are currently taking new patients.
Your first appointment
We offer every new patient a basic registration appointment so we can get up to speed with your Health and your medical journey. It means when problems arise we already have your background information. The appointment is chargeable, and can cover:
- Height, weight and BMI
- Blood pressure
- Your past medical history
- Your family medical history
- The medication you take now and any allergies you ahve
Booking appointments online
Once you are registered, you can ask for online access and book your own appointments or use the booking links on this website. If you wish to be able to request prescriptions online please complete the form below and return it to the practice. We can offer this to primary registered patients only.
Already registered with us?
If something has changed, send us an amendment form rather than registering again.
You can see some of the other clinicians of the Health Plus team without being registered with one of our doctors.
